For Referring Providers
We welcome referrals from medical providers, schools, therapists, dietitians, and community partners. Our team values collaboration and clear communication to support continuity of care.
To refer a client or patient, please contact our intake team using our main phone number, or fax. When possible, including relevant clinical information and level of urgency helps us respond efficiently and appropriately.
